Generally, 'medium' is used with sizes
Eg. I'd like a medium-sized portion please
Excuse me, do you have this t-shirt in medium?
(It also has another meaning: medium can mean something through which something is achieved eg. The train is a medium of transportion)

While 'middle' is used more when referring to place/location
He's standing right in the middle of the room
If you turn left, you'll see a sign in the middle of the road
I'm the middle child (in this case, if it was a family of three children, this person would be the 2nd oldest/youngest)
