It means that you can understand what someone is feeling - normally because you feel sorry for them a little bit too). The saying is quite comon and can be used in a range of situations!
For example, a friend could tell me she has an exam early in the morning and I could reply by saying "I feel your pain" because I can understand the difficulty of getting up early to sit an exam.
