"to" is describing something that is being given.
I'm giving this gift to Emily.
"for" is describing something that is being recieved.
This gift is for Emily.

You use both when your giving someone something, but you can use 'for' when you're giving to them directly, or giving to them by someone else, but you only use 'to' when you're giving something to someone by someone else. It might sound complicated but it's actually quite simple, for example;
'for':
'These chocolates are for you'
or:
'These chocolates are for her, will you give them to her?'
'To':
'Give this to him, tell him it's from me'

To is the direction something is going.
For is the location, person, or thing the direction is pointed at.
