All of them are almost the same meaning.
You just use it, we can get it. It's ok, no matter!
But they are little different:
Hiểu sai- it usually use for something, knowledge. Sometimes it use for human and it usually apparente in the test, books, news
Hiểu lầm- 100% it use for human. When someone try to explain that they are not like that (bad way, bad person or be wrong something )
Hiểu nhầm- this word is not common in the south Việt Nam. Ah ok, this word usually use in situations for example: you say something& mean something but i got something else, and that moment you said: bạn hiểu nhầm tui gòy, ý tui là vậy nè!
