No, for "eline sağlık" the person actually has to do something with their hands. There are similar but less common expressions for other body parts like "ağzına sağlık" e.g. when someone sings a song or says a prayer for you or "ayaklarına sağlık" e.g. when someone comes to visit you or runs an errand for you.
