That looks like an error to me. I would write the sentence in one of a number of ways:
English learners always say it's one of their biggest difficulties.
Every English learner says it's one of their biggest difficulties.
To me it sounds a bit 'over the top' to include both 'every' and 'always' in this sentence, and hence the following sounds a bit unnatural to me:
Every English learner always says it's one of their biggest difficulties.
