To answer your question directly: Yes, it is still properly formal to write "I was born June 12, 1970" (for example). A lot of English biographies start off this way.

Hi. "I was born April 9, 1999" can be used in any situation.
"I was born on December 21, 1945" can also be used in any situation, but is probably more formally used in biographies, etc.
That said, as you know, English doesn't have much difference between informal and formal versions, not nearly as much as other languages.
Regarding when you were born, there isn't really formal or informal versions, so you can use the above examples for anything.
