"How are you" in Japanese? I think it should be "お元気ですか?" for most cases. I have no idea to say it in different ways.

I see. Here you are as my opinion. お元気ですか is the most general one to say "how are you?" in Japanese. ご機嫌いかがですか sounds more formal for me. You wouldn't say it to your children, your close friends and your real parents. It also sounds a bit old-fashioned for me. Hope other natives will give their opinions.
