I feel like when you add "now" it is emphasizing the amount of time you have spent. It could be emphasis on various feelings, excitement, boredom, tediousness...
We've been together for 4 years now. (happy/surprised that the relationship is still going)
I've been watching videos for two hours now. (depends on context. maybe the emphasis is not that strong and you are simply stating that it's been 2 hours. maybe you lost track of time and you are upset you wasted time watching videos. maybe you are watching videos for work and it's taking much longer than expected.)
-You're still here at the library?
-Yeah, I've been studying for four hours now. (You can also say it without "now" and it means almost the same thing)
