“He has never been understood” is very general. Like: “Throughout his entire life, he has never been understood.”
“He was never understood” is past tense. Like: “When he was a child, he was never understood.” The present-tense version would be: “He is never understood.”
“He never be really understood” doesn’t make sense. You could say instead: “He is never really understood” which is pretty much the same as “He is never understood.”
