“It” is referring to being a doctor as a profession. It’s basically saying for every doctor who is just a doctor for the money, there is also a doctor that wants to be one because he/she cares about the patients.

@Critical_Thinker Yes. “In it for…” is a pretty common phrase, but what “it” refers to always depends on the context.
Most actors are in it for the fame.
You will never get promoted at McDonald’s if you’re just in it for the free food.
