A. 'to look after someone', means to take care of someone which can be for someone who's close to you!
Examples : My mother looks after the kids while I'm at work. (in this case, the relation is family bonds)
: Tell Joy that I sent you, and she'll look after you.
B. 'to look out for someone', means
i] being careful for oneself or for others:
Examples : Look out for your younger sister at school, and make sure no one bullies her.
: Look out for yourself when you're walking on the uneven road.
ii] while trying to spot someone/keeping careful watch for possible danger or difficulties.
Examples: Look out for children crossing the road. There's a primary school nearby.
: Look out for robbers when you're walking back home at night.
