It is an insult similar to "screw you!" or "f*ck off!" I think it's become less common, but that could just be my region and demographic.

Yes, not just an insult but a vulgar/offensive one, so it's not safe for common use. Say it to your boss and you might lose your job, say it to a random person and you could get into a fight.
