"We got a maniac on the loose." just means that there is a maniac on the loose
"We got us a maniac on the loose." implies that we are responsible for making sure that the maniac does not remain loose.

What nuance it gives when you add 'us'. I sometimes hear native speakers insert 'us' like that but I never add it because I don't know what it's for, exactly.

Us is used to take "ownership" of something. By using us in that example, he is indicating that you are going to become his problem to deal with.

I think us and ourselves could be used interchangeably in this context. I can't think of any examples where that would not work.

I believe more grammatically it should be "We('ve) got a maniac on the loose".
Basically both mean the same thing. Inserting "us" might be heard more often casually in the southern states. Some people also use it to mock a rural affectation as well.
