"What has gotten into you?" means "why are you behaving in this strange way?"
"What (has) happened to you?" can simply mean something happened to them and you don't know what. Maybe they are bleeding, or maybe their clothes are torn, you could use it to ask in any situation what has happened.

@EarleyGrave dices que, esto pasa cuando la persona actua raro psicologicamente, o lo encuentras molesto, o lo notas que su ropa esta rasgada, o le sucedio algo feo, en cualquer parte de su cuerpo?

@Anth22 You say the first one, "What has gotten into you?", when they are behaving strangely, not normally. Maybe they are being very violent or angry.
You say the second one, "What has happened to you?" when it's anything else, such as they are hurt or bleeding and something has *surely* happened to them (but you don't know what).

@EarleyGrave aya solo es cuando actua raro o molesto o timido como que te decepciona su comportamiento o ves algo en elde su ppersonalidad que te a miedo, anquesea enojado o lo que sea?

@Anth22 It usually means it scares or concerns you, but it could also mean they're just behaving in any strange way.

@EarleyGrave pero tambien se puede usar para cuaquier emocion que derrepente salga de la persona sin querer o que lo notas enjado, etc, y tambien aunque este muy alegre o feliz o , sol e cuando emociones negativas?

@Anth22 You say that when it looks like something must have happened to them recently; maybe they are injured, or their clothes are torn, or anything.

@EarleyGrave then what happened? se usa cuando ya se trata de otras cosas, y no emocionalmente, sino fisico, como caidas, o una fractura o herdida, , accidentes, etc. pero nada de emociones?
