@pannekoek The first can be taken very literal, or as a joke/casual. The second can be used to describe something/someone being hard to handle and/or annoying.

The first is casual.
The second is casual but rude. Ass is a rude word, do not use it with anyone other than friends!

"Pain in the neck" is more what you say to children or at work/school. "Pain in the ass" is very very casual, yes, because "ass" is a curse word
