To keep an eye on is to observe something/someone carefully
To keep an eye out for means the same thing ☺️

“To keep an eye out” means you are looking for something, but not actively. You are only watching to see if you happen to find it.
“To keep an eye on” means to watch something, for the purpose of making sure it isn’t going to do something you don’t want it to do
