In my mind at least, the difference is in how far you're going, how far they're going, and how many people are involved.
If you're seeing someone out, you're not going far, and it's not likely that they are either. I would see someone out if they're leaving my house after coming over for dinner.
If you're seeing someone off, you may or may not be going further, but they are going to be going a ways. I will be seeing my sister off when she leaves to move to Las Vegas, which is about 8 hours away from me.
To SEND someone off is usually reserved for a farewell party or mass goodbye from people that are SEEING someone off. You'll see people sending their loved ones off at the airport or on the docks as they go off for military service quite often, or when they go away to college.
These are by no means the absolute definitions, but it's the only way I can think of to differentiate.
