It depends. Not usually for me, but some accents are thicker than others. Also, sometimes slang in British English and "Ebonics" can be vastly different from that of "standard English," and grammar patterns in "Ebonics" can be fairly different too.
How easy it is to understand other dialects also has to do with exposure. For example if you were an English-speaking American kid, but watched a fair amount of British TV, British English would no doubt be easier to understand for you as opposed to someone who had no exposure.
Also, there are many, very different accents in Britain! Some more difficult to understand than others.
