If you put "really" first, it modifies "not hungry." So in that case, you are exaggerating the fact that you are not hungry. In other words, you are telling the person, "I'm definitely NOT hungry. 진짜 배가 안 고파.
If you put "not" first, it is modifying "really hungry." In this case, you are making sure the listener understands you are not very hungry, not really hungry, not dying of hunger. 그렇게 배가 안 고파요.
The first one is more emphatic, stronger. The second one is more mild.
