I am writing to introduce myself to you and your team. I am [Your Name] from Company A. I will be Mr. Z's replacement as the sales representative for your company and will be managing this account. (Note that Company C has a different account manager.)
I work out of Japan, but hope that we will be able to meet in-person one day.

Your message is mostly grammatically correct, but confusing. How many companies are you talking about here? I have a hard time following which company is which.
Here is my current understanding:
"I am replacing Mr. Z as your company (Company A)’s sales representative in Italy.
I work from Japan and am in charge of Company B, which is an Italian subsidiary of Company C. I will email Company B (the Italian company) to introduce myself."
Is Company C (the Japanese company) your company? Or another Japanese company?
Also, you are in Japan, right? Where is Mr. Z, Japan or Italy?

The company C is also our customer. C has many subsidiaries all over the world. However, I am in charge of only B (not C and other subsidiaries of C).
The point of my concern is “ sales representative in Italy” in the sentence.
Is the reader confused whether I work in Italy?
My co-worker have noticed that the reader get confused that I work in Italy as the agent of B company.
But I do not think so.

If you use your original version, some readers will get confused about whether you are in Italy or Japan. I think this should be a bit clearer:
I am replacing Mr. Z as the sales representative for your company (Company B?).
I work out of Japan and am now in charge of Company B's account. I will email Company B to introduce myself."

Your understanding is perfect.
I wanted to inform to the customer (company B) that I am in charge of only company B, not company C. However, I should not have written “in Italy” at the last sentence to avoid misunderstanding.
