When I ask someone " are you sick ? "
Does this mean I am asking him are u crazy ?

I would always think you meant "Are you ill" (or the opposite, "Are you healthy").
If someone does something very bad and seems crazy, people might say they are sick (or "sick in the head"). That is a phrase people use when they are angry, though. You would never ask someone if they are sick in this way.
